MILIA / MILIUM
A milium is for single lesion and multiple lesions are called milia. It is a small cyst containing keratin which is a skin protein produced by skin , the keratin entrapped beneath the outer layer of the skin froming a tiny cyst. This cyst is harmless and presented as a tiny pearl white bumps. It is formed at the base of the hair follicle or sweat gland. Milia are commonly found on the skin of people of all ages.
Chronic sun damage may also increase the condition. Long term use of cortico steroids creams and some cosmetics also aggravates the condition.
Milia are either primary or secondary. Primary milia are formed directly from entrapped keratin and are usually found on the faces of infants and adults.
Secondry milia are formed after something clogs the ducts leading to the skin surface, such as after an injury , burn or blistering of the skin.
CLASSIFICATION OF MILIA
Neonatal Milia — Often seen on the nose but may also arise inside the mouth on the mucosa or palate or can be more widely on scalp , face and upper trunk. It can be few to many lesions. It affect 40 to 50 % of new born babies.
Primary milia in children and adults –
Found around the eyes , cheeks , forehead and genetalia. In young children milia may appear along the nasal crease.
May be congenital or appear later in life.It is usually associated with Rombo syndrome , Basal cell naevus syndrome , Pachyonchia congenita , Gardner Syndrome and other genetic disorders.
Milia en Plaque
It affects children and adults especially middle aged women. Usually found on the eyelids , behind ears on cheek or jaw. Sometimes associated with other skin diseases including DLE , Lichen Planus and Pseudoxanthoma elasticum.
Multiple Eruptive Milia
Most affected areas are face , upper arms and upper trunk. Lesions are usually asymptomatic or itchy.
It arise from eccrine sweat glands. Occurs at the site of injury when skin heals.
Often seen on the back of hands and fingers in porphyria cutanea trada. Mostly seen in thermal burn , dermabrasion, blistering rashes like bullous pemphigus.
Very rare seen usually formed by use of topical medications e.g. Phenols , Hydroquinone , 5- Flurouracil cream , corticosteroids creams.
